Objectives: Osteosarcopenia, characterized by concurrent bone and muscle loss, poses a significant health risk in aging populations. This study explored the interactions between host genetics, gut microbiota, and musculoskeletal health, focusing on the therapeutic potential of microbiota modulation using Rg3, one of the phytochemicals known for its beneficial effect.

Methods: Using six genetically diverse mouse strains from the Collaborative Cross (CC) population, we evaluated the impact of Rg3 on osteosarcopenia and its microbiota-mediated effects. Two Eubacterium species, Eubacterium nodatum (EN) and Eubacterium ventriosum (EV), were identified as key mediators and their effects were tested in C57BL/6J (B6) and 129S1/SvImJ (129S1) mice.

Results: Rg3 treatment modified gut microbiota composition and osteosarcopenia-related phenotypes in a host genetics-dependent manner. EN and EV improved bone density, muscle mass, and exercise performance in 129S1 mice, reversing osteosarcopenic features via osteoblast differentiation and protein degradation pathways. In B6 mice, these microbes had limited effects on osteosarcopenia but influenced mitochondrial biogenesis and microbial diversity.

Conclusions: This study highlights the genetic dependency of gut microbiota interventions for osteosarcopenia. Strain-specific responses to EN and EV underscore the potential of personalized microbial therapies tailored to genetic backgrounds for effective treatment.
