(P19-052-25) Long-Term BMI Trajectories and Dyslipidemia, and Related Gut Microbial Features: Results From the China Health and Nutrition Survey 1991–2018

Objectives: Dyslipidemia is a major factor for cardiovascular disease, however, the association between long-term Body Mass Index (BMI) trajectories and dyslipidemia, as well as the intestinal and metabolic environment of body within different BMI trajectory populations remain unclear.

Methods: We chose adult individuals with more than three times of BMI measurement from China Health and Nutrition Survey (CHNS) 11499 individuals to conducted a group-based trajectory modeling, then we selected those who had two times of blood lipid measurements in 2015 and 2018 (n = 2003), and conducted a logistic regression to discover the risk trajectory group associate to future dyslipidemia. Then we selected two subsets in 2015 CHNS, CHNS-1 (n=3061) as discovery set and CHNS-2 (n=1409) as validation set to conduct gut microbiome analysis. Used LASSO regression and validated by logistic regression in both CHNS-1 and CHNS-2 to confirm the gut microbiome of each dyslipidemia risk trajectories. We also used a subset of CHNS-2 (n=779) which available for metabolomics data the adjusted p-value of Wilcoxon rank sum test combined with log2FC value to filtered out the differential metabolites between each group. Subsequently, we used spearman correlation analysis to discover the further connection between gut microbiota and metabolites.

Results: Three BMI trajectories were found in the study population, developed into Overweight (DO) group and Overweight to Obesity (OTO) group increased the dyslipidemia risk as compared to the Normal Stable (NS) group. For the gut microbiome analysis, alpha diversity decreased sequentially in DO group and OTO group. Setting the NS group as reference, we confirmed 6 genera in OTO group and 5 genera in DO group have lower abundance than NS group. We also found 36 and 5 metabolites that have differential concentration in OTO and DO group than NS which were mostly negatively associated with those predominant genera.

Conclusions: Chinese adults had three BMI trajectories with an upward trend over the long-term course and two BMI trajectory groups have a higher risk of dyslipidemia in the future. The dyslipidemia risk BMI trajectory may bring changes on overall gut microbiome signature and lipid metabolism.
